Premier will be glad to supply you with a copy of your medical records and x-rays. Please note that it can take up to a week to receive your records as some of our Care Centers use a copy service. To protect your privacy, you will need to submit a signed Medical Release Authorization ("HIPAA Patient Authorization form") to our office. In some cases, there is a cost associated for this service. Please contact the Care Center at which you are a patient for specific details.

For your convenience, you may download and print this form. You may then submit the completed and signed authorization form by mail or fax. If you personally pick up your records, be prepared to show your photo ID.
