Medications must be taken only in accordance with the instructions and dosage as prescribed by your Premier physician.

Prescription requests are handled only during regular office hours. When phoning in a request, please include the following information:

• Patient’s full name
• Patient’s date of birth
• Name of the medication
• Pharmacy name and telephone number

Be sure to specify if there is an emergency.

No narcotic pain medication prescriptions will be given when the office is closed.
